DIRECT BILLING POLICY:
Attention NEW CLIENTS with Weekend or Evening Appointments: If you wish to use our direct-billing services, we require coverage verification from your Extended Health Care provider. Insurance companies are closed evenings and weekends, please be sure to contact us on a weekday, ahead of your appointment, with your insurance information & date of birth.
If we do not receive your information prior to your appointment, we will be unable to direct bill for you.
Also please remember to arrive 10min before your appt. to allow time to fill out our Medical History form and our Health Authorization form. Please call us if arriving late.
